Mechanism
Cytokine binds receptor → receptor-associated JAK kinases (JAK1-3, TYK2) phosphorylate the receptor and one another → STAT transcription factors dock, are phosphorylated, dimerise, and translocate to the nucleus to drive transcription.
Why it matters for longevity
- Chronic inflammation: IL-6 → JAK1/2 → STAT3 is a canonical pathway of SASP amplification.
- Immune-cell aging: dysregulated STAT signalling underlies part of the immune-aging phenotype.
- Clinical translation: Baricitinib in NIA-funded frailty trials; ruxolitinib in graft-versus-host disease and myelofibrosis; tofacitinib in autoimmune disease.
Trade-offs
JAK inhibition carries infection, thrombosis, and cardiovascular signals in RA patients (ORAL Surveillance) — a real trade-off in longevity applications.
