What it is
An esterified form of D-β-hydroxybutyrate that hydrolyses in the gut to release free BHB into circulation. Achieves ketosis in carbohydrate-fed individuals within ~30 minutes.
Evidence
- Endurance: mixed. Some studies show performance improvement in ultra-endurance events; others null or negative.
- Cognition: acute BHB elevation improves cognitive performance in aged rodents and small trials in mild cognitive impairment.
- Cardiac: preclinical protection against ischaemia-reperfusion injury; small human trials in HFpEF ongoing.
- Inflammation: BHB inhibits NLRP3 inflammasome activation mechanistically.
Practical considerations
- Taste is famously unpleasant.
- Cost is high (single doses currently $10–30).
- GI intolerance common at higher doses.
- Not a substitute for the metabolic-flexibility benefits of endogenous ketosis from fasting or ketogenic diet.
